In short

How the Costco and Sesame GLP-1 program works in 2026: Wegovy or Ozempic for $349 a month, $199 intro fills, the Wegovy pill from $149 and care from $59.

Costco does not sell GLP-1 medication on its own terms. Instead, it pairs online care from Success by Sesame with discounted cash prices at Costco Pharmacy. Here is what it costs in 2026, who can use it, and how it compares with other ways to get a GLP-1 without insurance. Prices were checked against published Sesame and Novo Nordisk pricing in early October 2026.

How does the Costco GLP-1 program work?

You need two things: a prescription and a pharmacy. Success by Sesame, an online care marketplace, provides video visits with licensed clinicians who can prescribe a GLP-1 if it is appropriate for you. You then fill the prescription at a Costco Pharmacy at Costco’s self-pay price. If you already have a prescription from your own doctor, you can bring it to Costco Pharmacy instead.

In practice, someone on the Wegovy injection at a maintenance dose pays about $349 a month for the medication plus $59 to $99 a month for care, plus the Costco membership. Starting on the pill costs less at first and rises to $299 a month at the maintenance dose. See our Wegovy pill cost guide for the price at each dose.

Can you get Zepbound at Costco?

Zepbound is not part of Costco’s discounted GLP-1 pricing. The lowest cash price for Zepbound is through Eli Lilly’s LillyDirect, at $299 a month for 2.5 mg, $399 for 5 mg and $449 for 7.5 mg to 15 mg when refilled within 45 days, with home delivery or pickup at Walmart Pharmacy. Read compounded tirzepatide vs Zepbound for a full price comparison.

Is Ozempic at Costco approved for weight loss?

No. Ozempic is approved for type 2 diabetes, so prescribing it only for weight loss is off-label. Wegovy contains the same active ingredient and is the version approved for chronic weight management. Because the cash price is the same at Costco, Wegovy is usually the more appropriate choice for weight loss. Your clinician makes that decision.

Is the Costco GLP-1 program worth it?

  • Good fit if you are already a Costco member, want an FDA-approved brand-name medicine, and prefer picking up at a local pharmacy.
  • Compare first with NovoCare Pharmacy, which sells the same Wegovy pill and pen at similar self-pay prices with home delivery and does not require a Costco membership.
  • Check insurance before paying cash: if your plan covers Wegovy, the savings card can bring the cost down to as little as $25 a month.
