In short
Wegovy pill vs injection: OASIS 4 showed 13.6% weight loss with the daily pill, similar to the weekly shot. Compare side effects, dosing and cost.
By Alsen Walker · Updated October 6, 2026
Wegovy now comes in two forms: a once-weekly injection (semaglutide 2.4 mg) and, since the FDA approved it on December 22, 2025, a once-daily pill (oral semaglutide 25 mg). Both contain semaglutide. Here is how they compare on results, side effects, dosing and cost.
Is the Wegovy pill as effective as the injection?
In the OASIS 4 trial, 307 adults with obesity or overweight (without diabetes) who took the Wegovy pill lost an average of 13.6% of their body weight over 64 weeks, compared with 2.2% on placebo. Among people who stayed on treatment the whole time, average weight loss was 16.6%. In the STEP 1 trial, the Wegovy injection produced about 15% average weight loss over 68 weeks. The trials were not head-to-head, but Novo Nordisk describes the results as similar.
| Wegovy pill | Wegovy injection | |
|---|---|---|
| Active ingredient | Oral semaglutide | Semaglutide |
| How often | Once a day | Once a week |
| Maintenance dose | 25 mg tablet | 2.4 mg pen (7.2 mg Wegovy HD also available) |
| Key trial | OASIS 4: 13.6% average weight loss at 64 weeks | STEP 1: about 15% average weight loss at 68 weeks |
| Self-pay price (NovoCare) | $149 to $299 a month | $199 intro, then $349 a month |
What are the side effects of the Wegovy pill vs the injection?
Side effects are similar because the active ingredient is the same. The most common are digestive: nausea, diarrhea, vomiting and constipation, usually worst when starting or increasing the dose. In OASIS 4, digestive side effects were reported by 74% of people on the pill and 42% on placebo. Both forms carry the same warnings, including thyroid C-cell tumors in animal studies, pancreatitis and gallbladder problems. Read our side effects guide.
How do you take the Wegovy pill?
The pill is taken once a day, and the dose increases in steps, from 1.5 mg up to the 25 mg maintenance dose, as your clinician directs. Oral semaglutide must be taken in a specific way to be absorbed: on an empty stomach with a small amount of water, before food, drink or other medicines. Follow the instructions on your prescription label exactly. If a daily routine is hard for you, a weekly injection may be easier.
Which is cheaper, the Wegovy pill or the injection?
The pill is usually cheaper without insurance. Through NovoCare Pharmacy, it costs $149 a month at 1.5 mg, $199 at 4 mg and $299 at 9 mg and 25 mg. The pen costs $199 a month for a new patient’s first two fills, then $349. See the full breakdown in our Wegovy pill cost guide.
Should you choose the pill or the injection?
- The pill may suit you if you dislike needles, can take a tablet at the same time every morning on an empty stomach, and want the lowest self-pay price.
- The injection may suit you if you prefer a weekly routine, take other morning medicines, or want the higher-dose Wegovy HD option.
- Either way, decide with a licensed clinician who knows your medical history.
